PROfortil^TM Share Your Success Story Contest

Open to residents of Canada

1. CONTEST PERIOD:
   PROfortil^TM Share Your Story Contest commences at 10:00:00 AM on October 1st, 2019 and will end at 9:59:59 AM on December 31st, 2019. All times are Eastern Times.
2. ELIGIBILITY:
   To enter the Contest and be eligible to win the Prize a person must, at the time of entry, be a resident of Canada who has reached the age of majority in his/her province/territory of residence. Specifically excluded from entering the Contest are: (i) employees, shareholders, officers, directors, agents, representatives, and advertising and promotional agencies of the Sponsor, and each of their respective subsidiaries and affiliates; and (ii) a member of any such person’s immediate family (regardless of where they live) or persons with whom they are domiciled (whether related to the person or not). In these Official Rules, “immediate family” includes mother, father, brothers, sisters, sons, daughters, partner or spouse.
3. HOW TO ENTER:
   During the Contest Period visit https://conceptionplus.ca/contest and complete the contest Entry Form. Upon doing so, click “Submit” and your Story will be considered an official entry into the Contest. A person may enter once during the Contest Period.
4. ENTRIES:
   Limit of one (1) Entry per Contest Period per person. Entries generated by script, macro, robotic, programmed, or any other automated means are prohibited and will be disqualified.
5. FORMAT AND CONTENT:
   Each Entry must:
   • be in English or French;
   • have proof of purchase in the form of an attached document;
   • have proof of conception in the form of an attached document;
   • be no more than 500 words in length;
   • be sent by clicking the “Submit” icon on the Entry Form;
   • not contain any content that promotes any product or service other than that of the Sponsor;
   • contain entirely original materials that have never before been distributed, shown publicly or published, or selected as a winner in any other contest;
6. PRIZE:
   There will be a total of one (1) prize available to be won. The prize consists of one (1) UPPAbaby^® stroller delivered to the Prize winner as his/her name and mailing address appears on the entry form. The total value of the Prize is not to exceed $559.99 (CDN) before taxes and delivery fees. Prizes must be accepted as awarded without substitution, are not transferable, and not for resale. The Sponsor reserves the right, in the event that a Prize cannot be awarded as described for any reason, to substitute another prize of equal or greater value, without liability.
7. SELECTION PROCESS:
   By January 4th, 2020, one (1) eligible entrant will be selected from among all eligible Entries received during the Contest Period. The Selected Entrant will be determined by a panel of judges elected by Red Leaf Medical. The judging criteria is based on eligibility and the most compelling story shared. The odds of winning depend on the number of eligible Entries received during the Contest Period. The Sponsor will attempt to contact the Selected Entrant using the phone number and/or email provided in the Selected Entrant’s Entry within five (5) business days of the selection process. If a Selected Entrant does not respond to either phone call and/or email then he/she will forfeit his/her opportunity to be the Selected Entrant and the Sponsor reserves the right to select another entrant.
8. All Entrants agree that official entry of the contest hereby assigns full ownership rights and usage rights of story submission, first name, last initial and province provided by Entrants to Red Leaf Medical and Lenus Pharma. Red Leaf Medical and Lenus Pharma will have all rights to subsequent development, use, adaptation, and redistribution of contest entry submissions and their contents for advertising and marketing purposes. By entering the contest, entrants hereby waive any right of approval, any claim to compensation or benefit, any claim to further use of information provided by the entrant or anything derived from the information provided by the entrant, and any claim arising out of any use of the information provided by the entrant, other than as may be subsequently agreed upon in writing. Entrants agree not to upload any photos the provision, publication or use of which violates applicable law or violates the rights of third parties. Entrants expressly declare that they are in compliance with applicable data protection law. Each Entrant warrants that prior to disclosing personal information of any third party, it has obtained their consent and has ensured that the third party has received all information regarding privacy in this Participation Policy.
